    Meet the prerequisites

    Graduation from high school or its equivalent (required for entrance into the massage therapy school, per 481 IAC 822.2(5)). No numeric minimum-age requirement is stated in Iowa's massage therapy licensure rule (481 IAC 821); Iowa Code §147.3 (general health-profession qualifications) explicitly states an applicant is not made ineligible because of age. No fingerprint-based criminal background check requirement for original licensure appears in the current administrative rules (unlike some states); applicants seeking licensure by endorsement/reactivation must disclose any disciplinary action taken against a license in another jurisdiction. Iowa's own licensing page (dial.iowa.gov) separately states licensure requires U.S. citizenship or lawful-presence verification through the SAVE program. [5]

    Complete approved training

    Complete 600 hours of training at a state-approved program. Minimum 600 clock hours (or equivalent academic credit hours) at a board-approved school, per 481 IAC 821.1 and 822.2(5). Students must complete at least 200 hours of fundamentals/assessment coursework — including a minimum 100 hours of anatomy and physiology — before beginning the clinical practicum, and clinical practicum hours cannot exceed 25% of a program's total required hours (481 IAC 822.4). High school graduation or equivalent is required for entrance into the massage school. [1]

    Massage & Bodywork Licensing Examination (MBLEx)

    Pass the Massage & Bodywork Licensing Examination (MBLEx) (fee: $265). Iowa's rule (481 IAC 821.2) also accepts a passing score on a National Certification Board for Therapeutic Massage and Bodywork (NCBTMB) examination as an alternative to the MBLEx; results must be sent directly to the board from the testing authority. There is no separate 'application' processing fee distinct from the license fee — the $120 license fee (481 IAC 507.8(1)) is paid with the initial online application. The biennial renewal fee is $60 (481 IAC 507.8(2)).

Renewal & continuing education

Renew every 2 years. 16 continuing education hours required per cycle.

Licenses expire on the 15th day of the licensee's anniversary month every 2 years (481 IAC 821.1, 821.8). Continuing education is not required for a licensee's first renewal. Thereafter, 16 CE hours are required per biennium, of which at least 8 hours must be earned via in-person or live real-time interactive instruction (481 IAC 823.2-823.3).

Reciprocity

Endorsement/reciprocity from another state is available.

Per 481 IAC 821.4, Iowa grants licensure by endorsement to an applicant already licensed in another jurisdiction whose requirements are equal to or exceed Iowa's own (481 IAC 821.2), with direct verification of exam passage and current license status/disciplinary history from the issuing jurisdiction. A temporary license (up to 1 year, not renewable) is also available under 481 IAC 821.6 for an endorsement applicant still completing remaining requirements. Iowa has not enacted the Interstate Massage Compact (IMpact); as of this research the compact's enacted states are Nevada, Ohio, Arkansas, Virginia, and Montana, per massagecompact.org.
